The Appointment Procedure

In order to facilitate the appointment procedure, prior to scheduling an examination by telephone, you should have the following information:
� Name, address, telephone number, date of birth of the patient being scheduled.
� Insurance card, with the name of the insurance company, and the Social Security number of the member/employee covered by the insurance.
� Primary insurance information of dependent/spouse if covered by another insurance company.

When should I go to the Emergency Room?
East End Urgent & Primary Care is not always a substitute for your emergency room care. You should use a hospital's Emergency room for very serious problems.
If you are experiencing any of the following symptoms, don't wait! Call 911 or get to your nearest hospital emergency room:
� Cardiac chest pain
� Severe abdominal pain
� Severe difficulty breathing or shortness of breath
� Sudden dizziness, weakness or loss of coordination or balance
� Sudden blurred vision or loss of vision
� Numbness or weakness in your face, arm, or leg
� Sudden, severe headache
� Coughing up or vomiting blood
� Severe Burns
� Seizures

Urgent care is the prompt evaluation and treatment of most non-life threatening illness and injury.

East End Urgent & Primary Care is staffed with experienced emergency medicine trained physicians, physician assistants and nurses.

How long will I have to wait?
Patients are seen on a first come first serve basis. While every attempt will be made to see you as quickly as possible, wait times will vary on the number of patients and seriousness of your condition.

What forms of insurance and payments do you accept?

What happens after I leave Island Urgent Medical Care?
You will be advised about all follow-up procedures and notified about any abnormal lab work or X-rays. A complete record of your treatment will also be made available to your primary care physician, who can provide any necessary follow-up care.
